The School Climate Survey for Elementary students is a confidential survey designed to identify
student perceptions of the school climate within our school. Data collected will be used to help
us improve school safety, the quality of teaching and learning, and the strength of relationships
in our school.
The survey for grades K-2 includes 12 questions, and the survey for grades 3-5 is 20 questions.
The surveys should take no more than six minutes to complete and will be administered in your
students’ classrooms.
Responses to the survey will be housed securely in an anonymous format with the Utah State
Board of Education. All evaluation research projects are in compliance with the Family
Education Rights and Privacy Act, (343 CFR 99.31 (6)) and human subjects regulations
(Protection of Human Subjects 45 CFR 46). If you do not wish your students to take the survey, opt out forms are available in the office, and printable versions are linked below.
